UNIS laboratories

Protocol Deviation report:

A deviation report form must be filled out when an undesirable incident, damage to equipment, or personal injury has occurred in any of the UNIS labs.

The deviation may be reported to lab personnel, your immediate supervisor, your contact at UNIS or the UNIS safety delegate immediately following the incident or you can use the mailbox that is emptied every Thursday after 15:00 by the lab personnel.
