As recomendações para CDI nos pacientes idosos são semelhantes às indicações gerais de desfibrilador, desde que o paciente apresente expectativa de vida maior que um ano91:

Grau de recomendação I:

a) Ressuscitados de FV/TV ou de TV monomórfica hemodinamicamente instável de causa não reversível FE ≤ 35% (NE A).

b) TV sustentada com comprometimento hemodinâmico em portador de cardiopatia estrutural e FE ≤ 35% (NE A).

c) Prevenção primária em portadores de infarto do miocárdio há mais de 40 dias com importante comprometimento da função ventricular, FE ≤ 35% e classe funcional II e III, ou FE ≤ 30% e CF I, II, III (NE A).

d) FE ≤ 40%, TVNS espontânea e TVS indutível ao EEF (NE B).

Grau de recomendação IIa:

a) TV sustentada com comprometimento hemodinâmico em portador de cardiopatia estrutural e FE ≤ 35% (NE A).

b) Prevenção secundária em pacientes recuperados de PCR de causa não reversível, com FE ≥ 35% (NE A).

c) Síncope de origem indeterminada com indução de TVS hemodinamicamente instável (NE B).

d) Prevenção primária em pacientes com cardiomiopatia dilatada não isquêmica, CF II-III, com FEVE ≤ 35% e expectativa de vida de pelo menos um ano (NE A).

e) Prevenção primária em pacientes com cardiopatia isquêmica ou não isquêmica, CF III-IV, FEVE ≤ 35%, QRS ≥ 120 ms, para os quais tenha sido indicado TRC e expectativa de vida de pelo menos um ano (NE B).

Grau de recomendação III:

a) Presença de cardiopatia passível de correção cirúrgica ou percutânea (NE B).

b) Presença de cardiopatia isquêmica e FEVE ≥ 35% (NE B). c) Presença de taquicardia ventricular incessante (NE C).
